Nurse (Spinal Cord Injury)

The RMR Spinal Cord Injury and Disorders (SCI/D) Center specializes in:
I) efficient delivery of rehabilitation; 2) sustaining medical and surgical care; 3) patient and family education; 4) psychological, social, and vocational care; 5) research; 6) education; and 7) professional training in the continuum of care for persons with SCI/D. Patient populations served include:
a full range of care for all enrolled Veterans who have sustained a spinal cord injury or have a stable neurologic impairment of the spinal cord. Typical patient populations served include:
Spinal Cord Injury (SCI); Amyotrophic Lateral Sclerosis (ALS); Multiple Sclerosis (MS); as outlined by SCI/D VHA Handbooks and Directives. These patient populations are likely to present with complex conditions that may require acute stabilization; acute and sub-acute rehabilitation; acute and sub-acute medical and surgical care; preventative care; respite care; hospice care, as appropriate. The SCI NCM provides age-specific care for adult (18-64), older adult (65-74) and geriatric (over 75) patients who have sustained damage to the spinal cord throughout the continuum of their lifetime, which incorporates stabilization after injury, rehabilitation, unrelated medical/surgical conditions, health maintenance and long-term follow-up of the injured individual, as well as psychosocial support including the family of significant others. The NCM promotes achievable independence and adjustments conducive to health, assisting the patients and their families to look beyond the disability. Their supportive interventions reduce the stigmas of disability and help individuals reestablish and maintain control over all aspects of their lives. The NCM coordinates and collaborates with interdisciplinary teams to promote efficient, effective, individualized care to assist the patient in achieving optimal function and health. Members of the interdisciplinary team vary depending on the needs of the patients. The NCM promotes the VA Mission, Core Values, Strategic Goals and Objectives. Additionally, the SCI NCM upholds the Mission of the Spinal Cord Injuries and Disorders program in VA:
To support and maintain the health, independence, quality of life, and productivity of Veterans with spinal cord injuries and disorders throughout their lives. The NCM provides evidence-based care and functions as a coordinator of patient care for inpatient and/or outpatients in the SCI/D Center. The NCM collaborates with the patient/family and the interdisciplinary health care team with the goal to reduce admissions, inpatient days, decrease length of stay, and reduce number of urgent care visits. The NCM assesses, plans, implements, coordinates and evaluates the holistic care of patients enrolled in the SCI/D, ALS and MS registries. The NCM will coordinate the complex physical and psychosocial needs of these patients. The NCM uses advanced clinical, interpersonal, and communication skills to assist patients/families to use services appropriately, to enhance the continuity of care and to achieve desired patient outcomes. The NCM duties include facilitating the coordination of care for the veteran population assigned to staff SCI/D physicians and/or other providers as appropriate; relaying information between the patient and team members; coordinating care among services and across the continuum for veterans with complex, physical and psychosocial needs assigned to staff SCI/D providers; coordinating SCI/D Center admissions; discharge planning; annual evaluations, clinic appointments, SCI/D lodging, medical home care needs, and SCI/D Telemedicine; providing professional nursing assessment and intervention to patients with episodic or chronic care needs; providing and coordinating patient education activities to enhance patient movement toward self-management; educating patient and family regarding current risk factors and the benefits of lifestyle changes for optimal bio-psycho-social functioning; facilitating use of preventative, wellness and health promotion activities to enhance overall health status; mobilizing patient and family to employ healthy coping strategies, engage in shared decision-making and utilize VA/community resources; facilitates fluid coordination of care for SCI/D Center hub/s and outlining spoke sites; coordinates spoke site and or other SCI/D center admissions, transfers and discharges; coordinates and collaborates in day to day SCI/D center operations; fulfills the mission and vision of RMR SCI/D center, SCI/D National and the overall VA Mission completing defined duties and other duties as assigned. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. Mechanical inpatient lifts are provided. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements as a condition of employment. The incumbent who provides care in home settings or other off-site locations may be required to drive and/or ride in GSA-vehicles.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
